THIS FORM IS FOR CAR REGISTRATION FOR ALL PAMC PARKING LOTS.

A separate form must be filled out for each stall.

Please note there is a $25 Refundable deposit due for PAMC Parking Lot decals. Once your stall has been terminated at the end of your term and your PAMC decal has been returned in good condition your deposit will be refunded to you. Your PAMC parking decal will not be issued to you until your refundable deposit has been received by our office.
